U.S. Department of Labor Cites U.S. Virgin Islands Concrete Factory
For Exposing Employees into Crushing Hazards Following Fatality

GUAYNABO, PR — The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) has mentioned High Quality Concrete — established in St. Croix, U.S. Virgin Islands — for both crushing and other dangers following a worker fatality on Sept. 6, 2019. OSHA cited the concrete mill for deliberate, serious and other-than-serious offenses, with proposed penalties of 90,217.

The worker suffered fatal injuries following the forklift that he had been having to lift and move a 3,200-lb bag of cement overturned because of an unstable load. OSHA mentioned High Quality Concrete for not training forklift operators about how to properly lift and transfer loads, and neglecting to make sure that operators understood how to take care of stable loads and wore seatbelts while managing forklifts.

Following that the fatality, OSHA inspectors returned to the mill three times and discovered that High Quality Concrete always failed to make certain that employees managed safely or safely arranged loads, leading to the intentional violation. OSHA issued an additional citation for the company’s failure to report that the worker fatality within 8 hours.

“Proper training on the safe use of a forklift could have prevented this tragedy,” stated OSHA Puerto Rico Area Director Alfredo Nogueras.  “Identifying hazards and implementing best practices during the load handling process can protect workers from serious and fatal injuries.”

High Quality Concrete has 15 business days from receipt of those citations and penalties to comply, request an informal conference with OSHA’s area director, or contest the findings prior to the individual Occupational Safety and Health Review Commission.
